Social Prescribing is a Supportive and Holistic Approach to working with those most vulnerable in our Communities. It is for:
- Adults over 18 who may be impacted by, a long term Health Condition
- Those who are frequent attendees of Emergency Departments or GP services.
- Those who struggle with their Mental Health
- Those who have complex Social Needs that impact their health and wellbeing.
It is a collaborative approach with Community, Voluntary and Statutory agencies, offering the best outcomes for those who are at most risk of social isolation and loneliness.

What is Social Prescribing?
Social Prescribing is an initiative supported by the HSE, it is a supportive and holistic approach to healthcare that can help link individuals with health and social resources in their community.
Who is Social Prescribing for?
It is for anyone over 18 years of age who are impacted by a long term health condition and want support in engaging with services in their area. Individuals who are struggling with their Mental Health, who are lonely or isolated and need support in accessing supportive services in their community. Those that have complex needs that impact their health and wellbeing.
Where does the support take place?
Initial support is offered at the most convenient place for the referee to access? The social prescriber endeavours to uphold confidentiality and has access to quiet spaces to carry out assessments within their catchment area
How much is the support?
There is no fee to access the support, Support is offered for 6-8 sessions.
